Event Request Form

Please use the attached form to request the use of a facility and publish it in the bulletin and/or website.

To fill out the form just follow these easy steps:

1. Save this file to your hard drive using a unique file name (e.g. soup_supper_march08.doc).
2. Use your mouse to place the cursor in the gray boxes and fill in the required information. You may also use the Tab key to move between fields.
